our memberships offer
your own dedicated RN: each member is paired with a CA licensed registered nurse who services as your personal care guide & trusted point of contact for you and your family
personalized care planning & coordination: your nurse helps organize health-related information, supports care planning, and coordinates communication with physicians and other licensed providers as appropriate
health education and preventive guidance: receive evidenced-based education focused on prevention, wellness and supporting the management of chronic health conditions that align with physician-directed care plans
ongoing nursing support services: members have access to ongoing, non-emergent nursing support, clinical guidance and patient education that falls in line with the RN scope of practice.
